My Health

Medical

You and Calpine share the cost of this benefit. Medical coverage is offered to you and your covered dependents.

Prescription Drug

This benefit is automatic when you enroll in the medical plan. Coverage includes generic, preferred brand and non-preferred brand medications prescribed by your doctor.

Health Reimbursement Account (HRA)

Calpine makes contributions to this account – and you can earn additional HRA dollars if you participate in the Calpine Wellness Program.

Wellness Program

Calpine offers a suite of programs to help you and your spouse/domestic partner get (or stay) healthier, including disease management, telephonic wellness coaches, health trackers and much more.

Dental

You and Calpine share the cost of this benefit. Dental coverage is offered to you and your covered dependents. Coverage is provided for preventive, basic, major services and orthodontia.

Vision

You and Calpine share the cost of this benefit. Vision coverage is offered to you and your covered dependents. Coverage is provided for regular eye exams, lenses, frames and contacts.

Health Care and Dependent Care Flexible Spending Accounts (FSAs)

Calpine provides you a way to save money on a before-tax basis for eligible out-of-pocket health care and dependent day care expenses (for you and any dependent you claim for tax purposes).

Eligible health care expenses include most medical, prescription, dental and vision expenses that plans do not cover. See www.myuhc.com website or www.irs.gov (Publication 502) for a list of eligible expenses.

Generally, eligible dependent care expenses include day care for dependents that allows you to work. See www.myuhc.com website or www.irs.gov (Publication 503) for eligible dependent care expenses.

Short-Term Disability (STD)

Calpine pays the cost of this benefit. STD replaces a portion of your income for up to six months if you are unable to work due to injury or illness.

Long-Term Disability (LTD)

Calpine pays the cost of this benefit. LTD provides 66.67% of your monthly covered earnings up to $15,000 per month if you have a qualified disability.

Basic Employee, Spouse/Domestic Partner and Child(ren) Life Insurance

Calpine pays the cost of this benefit. Basic Life Insurance provides financial protection to you and your beneficiaries if you, your spouse/domestic partner or child(ren) should die.

Basic Employee Accidental Death and Dismemberment (AD&D) Insurance

Calpine pays the cost of this benefit. Basic AD&D Insurance provides financial protection to you and your beneficiaries if you should die or become disabled due to an accident.

Voluntary Employee, Spouse/Domestic Partner and Child(ren) Life Insurance

Voluntary Life Insurance provides additional financial protection to you and your beneficiaries if you, your spouse/domestic partner or child should die.

Voluntary Employee Only or Employee + Family Accidental Death and Dismemberment (AD&D) Insurance

Voluntary AD&D Insurance provides additional financial protection to you and your beneficiaries if you, your spouse/domestic partner or child(ren) should die or become disabled due to an accident.

Employee Assistance Program (EAP)

Calpine pays the cost of this benefit. The EAP provides confidential assistance to resolve problems that are affecting you, your job and your family.

Adoption Assistance

Calpine provides you with financial support when adopting a child(ren).

Calpine allows you to save money that is tax-deferred and receive a company matching contribution on the first 5% of your contributions.

Commuter Expense Account

Calpine allows you to pay for your transportation expenses with pre-tax dollars. Calpine pays the first $80 of your mass transportation costs per month.
